Turning in Forms to the Trainer: Specific instructions on the final steps for medical clearance.

To ensure your Lady Bearcat is eligible to play on day one, we are collecting all medical and participation documents at this meeting. Our Athletics Trainer will be on-site to process these immediately.
Please bring the following completed documents with you:

Completed Physical Form: Must be signed by a physician.

Required Participation Paperwork: All signatures must be present.
Direct Submission: Be prepared to hand these forms directly to the Trainer during the designated time in the meeting.
